MICS Questionnaires The survey unit is a household: A person or a group of persons living and eating together HOUSEHOLD QUESTIONNAIRE Usually for a sub-sample of households QUESTIONNAIRE FOR INDIVIDUAL MEN (AGE 15-49) QUESTIONNAIRE FOR INDIVIDUAL WOMEN (AGE 15-49) QUESTIONNAIRE FOR CHILDREN UNDER-5 (AGE 0-4) Questionnaire Form for Vaccinations at Health Facilities

Interview and eligibility Household Questionnaire Interview a knowledgeable adult member, obtain list of household members Women’s Questionnaire Interview all women age 15-49 Men’s Questionnaire If household selected for men’s questionnaires Interview all men age 15-49 Under-5 Questionnaire Interview mother If mother is not a household member, interview primary caretaker
