CROWN Web will . . . • Require data entry by facility staff • Be Web-based • Be patient-centered
CROWN Web will require data entry by facility staff • SYSTEM REQUIREMENTS • Windows XP (SP2) or Windows Vista • Internet Explorer 6 or 7 • Adobe Acrobat Reader 8 or higher • High-speed/broadband Internet connection
CROWN Web will require data entry by facility staff • STAFFING REQUIREMENTS • Understanding of facility’s processes • Computer literacy • Security awareness • Specific CROWN Web training • Time to do the work
CROWN Web will requiredata entry by facility staff • ADMINISTRATIVE REQUIREMENTS • Analyze the facility’s flow of information • … and how it may need to change • Identify who will be CROWN Web users • Specify the roles and responsibilities of users • Designate a Security Administrator
What about Large Dialysis Organizations? • Eventually, all data will be submitted directly from corporate systems – “Batch”. • Initially, only data currently submitted on PARs and CPMs will be submitted via batch. • Initially, 2728s and 2746s will be done through the single-user interface.
Can other facilities use batch transmission? Eventually -- Yes Initially -- No
What about transplant facilities? At least initially, transplant facilities will submit the current paper forms Data will be entered into CROWN Web by Network staff
What about VA facilities? A required data use agreement between CMS and the VA is pending. Until then . . . At least initially, VA facilities will submit the current paper forms Data will be entered into CROWN Web by Network staff
CROWN Web will be Web-based • No software to install (Well, hardly any) • Available anywhere • Reduced risk of inconsistent data • Efficient system updates BUT . . . • Security and privacy are major concerns

CROWN Web will be patient-centered • “Admission” data will precede forms • Forms will be pre-populated as much as possible • General clinical data will precede CPMs • No more PARs. However . . .
Clinical Data in CROWN Web • Clinical measures required on all patients • Clinical measures to be updated monthly
Patient Activity Reports (PARs) to be replaced byPatient Attributes and Related Treatments (PART) • PART is a summary of a facility’s patients • If a patient’s record has received no updates • during a month, check the ‘No Changes’ box • Shortcuts will be provided
Other Tidbits • Facility data will also be in CROWN Web. • Networks will initiate • Facilities will complete and maintain • Includes key personnel • The language of “Events” will be replaced • by the language of “Admits” and “Discharges”

Data Transition Calendar • 11/5/2008: October PARs due • 11/17/2008: Rosters and Surveys mailed to facilities • 12/3/2008: Completed Surveys due to Network 11 • 12/5/2008: November PARs due • 12/15/2008: Facility Surveys faxed back to facilities • 1/5/2009: Last day to submit 2008 forms and PARs • 1/16/2009: Last day Network 11 can enter data
