Affidavit of Provisional Voter Texas Secretary of State
Affidavit of Provisional Voter • Any voter that does not appear on the list of registered voters, does not have acceptable identification, or whose eligibility to vote is challenged should be offered a provisional ballot. • The voter must execute the Affidavit of Provisional Voter (front of the green envelope) . • The Affidavit of Provisional Voter serves as a registration document.
Affidavit of Provisional Voter • After the election, the provisional ballots will be transferred to the Voter Registrar to review voter’s eligibility. • The Voter Registrar will make a copy for voter registration purposes and return original to the General Custodian after the Voter Registrar has concluded his/her review. • The voter is registered whether or not the voter’s provisional ballot was counted. Application for Limited Ballot Texas Secretary of State
Application for Limited Ballot • Allows a registered voter (active & suspense), who has moved from their county of registration, and does not have effective registration in their new county. • A limited ballot may ONLY be voted during early voting – in person or by mail. • Early Voting Clerk must verify the applicant does not have an effective registration in the new county prior to providing a Limited Ballot. • The voter is only entitled to vote for statewide offices, common districts, and propositions. Application for Limited Ballot • Completion of the limited ballot application will serve as a registration application for future elections in the county in which the application is submitted. • Early Voting Clerk must forward a copy of the Limited Ballot to the prior county voter registrar not later that the 30th day after receipt of the Application for Limited Ballot. • Use form “Notice to the Voter Registrar of Application for Limited Ballot.” Texas Secretary of State

Voter Registration Address Confirmation • The Address Confirmation Notice informs the voter that if he/she completes the response form and indicates he/she no longer lives in the county, the voter registrar will forward the Address Confirmation Response form to the new county of residence, which will serve as a voter registration application in the new county. • The voter registrar can use the Address Confirmation Response Notice form to cancel the voter’s record if the voter has indicated an address outside the county. The response form should be forwarded within (2) working days to the new county. Texas Secretary of State

FPCA Application Texas Secretary of State
FPCA Application • For Overseas Voters who are eligible to vote in Texas elections. • For Military personnel and their families stationed with them. • Applicant automatically receives ballots for which they are eligible. FPCA application valid for (1) one calendar year. • Received by the Early Voting Clerk by mail, common or contract carrier, fax, or email scan. • Serves as ballot application and voter registration document. FPCA Application • The ABC Process: • A (Application)Fill out and file your FPCA (Federal Postcard Application) as soon as possible but no later than a week before Election Day. • B (Ballot)Receive your ballot or use the FWAB (Federal Write-in Absentee Ballot) • C (Casting and Counting)Cast your vote by returning your marked ballot by Election Day, or the 5th day after if voting from overseas. Statement of Residence Texas Secretary of State
Statement of Residence • Voter is provided a Statement of Residence when they appear at the polling place and indicate that their address has changed, or if they have an “S” beside their name on OLRV. • The Statement of Residence form reflects the same information on the voter registration application, which allows the form to act as a voter registration application (or an update to their voter registration status). • When a new address provided on the Statement of Residence is in a Texas county other than the one in which the voter is offering to vote, the voter registrar can use that statement to cancel the voter’s record by copying the statement for their record, and forwarding the statement to the voter registrar in the new county. The new county voter registrar will be able to use the statement as a voter registration application. Texas Secretary of State

www.texas.gov • This paperless Web application is when a voter is making a name/address change within their present county. • If an Offline County receives a TX Online Data application that is not in their county, the county will make a copy and forward to correct county. • If an Online County receives a Web application that is not in their county, the county will reject the application, and cancel the voter since they no longer reside in the county. • The prior county sends a copy of the Texas Online application to the correct county. The new county can mail a voter registration application to the voter since the Texas Online Application can’t be treated to send a Notice of Incomplete. Texas Secretary of State

SOS Online Application • On the SOS home page click on “VoteTexas.gov” – Voter Information. • On the VoteTexas.gov webpage click “Register to Vote” at top of page. • Then click on “Get Your Application Here.” • Informal online application (can be filled out online, but must be printed, signed, then mailed). • Receiving a postage paid application in the mail.
