2. Survey Location – Manchester, MD • Manchester is one of eight Incorporated towns in Carroll County, MD • Located in the Northeast corner of Carroll County, bordering Pennsylvania, Baltimore County, and the towns of Hampstead and Westminster • The water supply for Manchester is either public water from the Town of Manchester or Well Water: • Only a few reports of contamination have been reported over the past ten years. • Primary Heating Sources utilized: • Electricity (43%); Fuel Oil (34%); Gas (19%); and other heating sources (4%).

3. The Community • The town of Manchester is a Rural community that has a farm base with new residential communities in recent years: • The current population is just under 5,000 residents, however based on data from City-Data has grown by 44.5% since 2000; • In the past 13 years there have been at least three housing communities built in the Manchester area, bringing new residents to Carroll County, however the infrastructure seems to meet the needs of the growing community; • The median age is 37 years old, with a community that is 51% Female and 49% males; • The marital status in the community is stable at 65%, yet 19% have never married, 9% divorced and/or separated and 7% widowed;

The Community – Continued • The community of Manchester is not highly diverse in culture; over 90% of the residents are white; 4% are Hispanic; 3% are Black and the remaining 3% are classified as Other; • Manchester is an Educated community with over over 80% having a High School diploma or higher education, 15% have a Bachelor’s degree and 4% have a Graduate degree; • Unemployment is lower than the State average at 6.5%; • There are two Elementary Schools, and a High School in the Manchester community and a Middle School in the neighboring community; all of the schools with the exception of one Elementary School have been built in the past decade; • Community recreational facilities are available and adequate ro promote an active community.

5. Environmental Health • The most significant factor with regard to the Environment is the Radon Screening rating of 4pCi\L (Pico curies per liter) • The Air Quality Rating for Manchester is above the National Average, with a 35.2 rating for Manchester and the National Average of 32.0, in this case lower is better, while it is higher it’s not a risk level for the community;

6. Access to Health Care and Health Statistics • Manchester is a small town, rural community with it’s own small Main Street; however, there are a number of small Medical practices in town, and within a ten mile radius in nearby Westminster all medical specialty practices as well as Carroll Hospital Center; • There are many other hospitals within a 40 mile radius, including University of Maryland Medical Center aka Shock Trauma and Johns Hopkins among others; • The statistics for births and deaths are consistent with Maryland State averages and percentages; • The statistics regarding cause of death are also consistent with that of the State with regard to the top three causes.

7. Windshield Survey • Implementation of Pyxis Point of Care Verification System • The Point of Care Verification System works in conjunction with the existing Pyxis Inventory Management System • The change would include scanning of patient armbands • Scanning the medication that was retrieved from the Pyxis Inventory Management system • The system verifies that the correct patient is about to receive the correct medication (both medication and dosage) • Upon verification from the system the healthcare provider can administer the medication to the patient • The system will electronically record the time and date of medication administration and the provider who administered the medication.

Community Health Diagnoses – Cont’ • My overall Health Diagnoses for Manchester, MD is above average; this is based on a compilation of facts; the Community has grown, the infrastructure is able to support the growth, the community is stable financially and environmentally; and Carroll County Health Department has a Local Health Plan that is aligned with the Healthy People 2020 project and is actively working in the community to achieve it’s goals and objectives.

10. Top Three Problems Identified • Despite the fact that I consider this community to be above average, I was still able to identify three top health issues that need to be addressed: • 1 - Overall Health, Fitness and activity of the community members; • 2Despite the fact that I consider this community to be above average, I was still able to identify three top health issues that need to be addressed: • 1 - Overall Health, Fitness and activity of the community members; • 2 – The Use of Tobacco, Alcohol and other substances; and • 3 – Implementation of Radon Mitigation systems in new homes. • – The Use of Tobacco, Alcohol and other substances; and • 3 – Implementation of Radon Mitigation systems in new homes.

11. Top Three Problems Identified • Despite the fact that I consider this community to be above average, I was still able to identify three top health issues that need to be addressed: • 1 - Overall Health, Fitness and activity of the community members. This issue has the potential to impact the physical health of those in the community, including Heart health which is the number one cause of death in Maryland and Carroll County; • 2 – The Use of Tobacco, Alcohol and other substances. The use of Tobacco has been a long standing health issue contributing to the lung and heart health; the Surgeon General has a top initiative to Prevent Tobacco use among young people, and illegal use of substances and alcohol underage continue to be community issues; • 3 – Implementation of Radon Mitigation systems in new homes. With the rating of Highest Risk for this community, and Cancer deaths on the rise this is a issue that needs to be addressed, which can only have a positive impact on this growing community.

12. Problem #1 – Goals and Objectives • #1 - Goals and Objectives to Increase the Overall Health and Fitness of the individuals in the community; • Objective: % of adults who exercise 30 minutes or more at least 3 times per week • Goal: 40% Measured – via Surveys • Objective: % of children who exercise or participate in recreational activities for 30 minutes or more 4 times per week • Goal: 50% Measured – via Surveys • Objective: % of people (children and adults) who are technically classified as Obese • Goal: 7% or less for Children; 15% or less for Adults Measured – medical reporting

Problem #2 – Goals and Objectives • #2 - Goals and Objectives related to Smoking, Drinking and Subtance Abuse in teens and Young Adults: • Objective: % of teens in High School whom smoke • Goal: 15% Measured – via Surveys • Objective: % of teens in High School and College under the age of 21 who reported to have consumed Alcohol at all over a 30 day period • Goal: 10% Measured – via Surveys • Objective: % of teens in High School and College who reported to have used any drugs over a 30 day period • Goal: 10% or less Measured – via Surveys

Problem #3 – Goals and Objectives #3 - Goals and Objectives for regulations regarding the Radon Detection systems: • Objective: Pass legislation at the local or state level to requirement Radon Mitigation Systems • Goal: Within 2 years Measured – via Enactment • Objective: Gain approval for tax deductions at state level for volunteer implementation of a home Radon Mitigation System • Goal: Within 2 years Measured – via Enactment

15. Maintaining a Quality Community Health Assessment and Diagnoses Impact on the Community Increased quality of health and potentially a longer life for the citizens of this community; Changes for our Youth that will be a benefit to their health; We need to be proactive and support the Surgeon Generals campaign to prevent the use of Tobacco among youth and young adults; Implementation of systems that may reduce the risk of exposure to Radon and other carcinogens that cause Cancer; Maintain Quality Services, continue to grow the community and Maintain a Quality Reputation. • Manchester is a growing community with quality services both within the community and within the County; • The Schools, Recreation Facilities, Highways and Roads and other infrastructure are current and providing adequate support to this community; • The areas of improvement sited in the Heath Diagnoses include Physical Fitness, Health and Activity for community members; Use of Tobacco, Alcohol and other substances; and Implementation of Radon Mitigation. The first two items are common to almost to almost all communities in the US and as a nation we are beginning to see improvement. • Manchester is part of Carroll County, MD, this county has identified and implemented a plan to address many factors within the Healthy People 2020 program.
