  2. Personal data : Female,20 years, from Suhag Married 2 years ago, no children Pregnant 5th month No special habits -ve family history

  3. Presentation • Referred to NCI as a case of Rt. Suprarenal mass • Explored outside NCI, resection failed due to bleeding

  4. Present history : • Accidentally discovered,u/s pregnancy follow up • In retrospect, the patient reported progressive abdominal heaviness and distention 2month • No symptoms of functioning adrenal tumors

  5. Past history : • Lt ovariectomy ,10 month ago u/s 7 x10 cm ,solid and cystic immature teratoma NO FORMAL PATHOLOGY REPORT

  6. Examination : • General :Pale, toxic • Abdomen :large solid mass Rt. Side, restricted mobility .

  7. Investigation :labs. • HB 6 g/dl • TLC 14,ooo cell/microlitre • LDH 959 u/l • AFP 850 ng/ml • BHCG 26719 mIU/ml

  8. Investigation :imaging (MRI)

  9. Decision : Exploration

  10. Operative findings:

  11. Postoperative : • Uneventful • Fetus viable • Discharged 10 days postoperative

  12. Pathology : • Hepatoblastoma

  13. Revision of pathology : • Metastatic immature teratoma

  14. Adjvuvant ttt : • Immature teratoma : CTH after the end of pregnancy. • If hepatoblastoma ????
