Presentation • Referred to NCI as a case of Rt. Suprarenal mass • Explored outside NCI, resection failed due to bleeding
Present history : • Accidentally discovered,u/s pregnancy follow up • In retrospect, the patient reported progressive abdominal heaviness and distention 2month • No symptoms of functioning adrenal tumors
Past history : • Lt ovariectomy ,10 month ago u/s 7 x10 cm ,solid and cystic immature teratoma NO FORMAL PATHOLOGY REPORT
Examination : • General :Pale, toxic • Abdomen :large solid mass Rt. Side, restricted mobility .
Investigation :labs. • HB 6 g/dl • TLC 14,ooo cell/microlitre • LDH 959 u/l • AFP 850 ng/ml • BHCG 26719 mIU/ml
Decision : Exploration
Postoperative : • Uneventful • Fetus viable • Discharged 10 days postoperative
Pathology : • Hepatoblastoma
Revision of pathology : • Metastatic immature teratoma
Adjvuvant ttt : • Immature teratoma : CTH after the end of pregnancy. • If hepatoblastoma ????
