Exposure to herbs containing Fangji increases the risk forrenal failure- a nested case-control study in Chinese herbalistsHsaio-Yu Yang, Jung-Der Wang, Tsai-Chang Lo, Pau-Chung ChenInstitute of Occupational Medicine and Industrial Hygiene, National Taiwan University College of Public Health Aristolochic acid (AA) is a nephrotoxic agent and is a common ingredient in many Chinese herbs, especially Menispermaceae (Fangji in mandarin). Chinese herbalists had been chronically exposed to herbs and were found to have higher mortality risk for chronic kidney disease. This study aimed to clarify whether the exposure to herbs containing AA increase the risk for renal failure among herbalists. Background & Objective Results & Discussions Table 2: Characteristics of cases of renal failure and matched controls Materials & Methods Study population consisted of 6,538 herbalists who were registered in the Chinese Herbalist Union between 1985 (when the union established) and 1998. With a nested case-control design, we followed the development of renal failure from the Catastrophic Illness Registry of the Bureau of National Health Insurance. All newly diagnosed cases of renal failure during 1995 to 2000 were defined as case group. Among the individuals who did not have renal failure, up to 4 controls were individually matched to every case by gender and year of birth (less than 5 years older or younger). With a semi-structured questionnaire survey by interviewers, we obtained the demographic data, occupational, environmental, lifestyles and past medical histories. Data was analyzed with conditional logistic regression to assess the morbidity odds ratio. P value was calculated by Chi-square test and independent t- test. Table 3:Association between renal failure and Chinese herbs exposure ¶Adjusted for age # Adjusted for age and use of analgesics ≧ 1 /week Conclusion: occupational exposure to herbs, especially Fangji, and chronic taking Fangji might increase the risk for renal failure among Chinese herbalists.
