    4. Health History Form This form is in the Guide to Academic Success booklet that students have received. Form must be completed and returned to Hudson by September 1, 2006. Actual dates of immunizations are needed. Add notes from your physician if needed.

    5. Immunizations None are required. Meningitis and Hepatitis B are recommended. Meningitis immunization is available at Hudson from 12:30 PM to 2 PM for Precollege students. A vaccine clinic for Meningitis and Influenza will occur in late October.

    6. Fees and Charges There is no fee to see a physician or nurse at Hudson, however, there are charges for ancillary services. Any charges go on the students OU account. At the time of treatment, the student will be given information on paper that parent can use to bill the insurance company.

    7. Remember Confidentiality Emergency Care First Aid Kit Copy of health insurance card and information about any special requirements for students covered by HMO, PPO, etc.

    10. Accident and Sickness Insurance Policy All students are required to have a health insurance plan while they are enrolled in the University. All students are automatically billed for a health insurance plan offered by Ohio University. Domestic students who are covered by an insurance plan may waive the University-offered plan; you will do this electronically when you receive your electronic fee bill. You must waive insurance by October 9th. Visit the Bursars table or Chubb Hall 015.

    16. Enrollment Verification Once your student is registered, our office can provide an enrollment verification letter Many times your insurance companies will ask for such letters to continue coverage for your student

    17. FERPA Family Educational Rights and Privacy Act of 1974 Guarantees privacy of student records. Any institution receiving federal funds must comply. FERPA rights transfer to the student when attending a postsecondary institution, regardless of age Student must give written permission to the University to release confidential information Grades and schedules are confidential information

FERPA We strongly encourage communication between the student and you: Grades and schedules are emailed to the student If you are interested in knowing this information, ask (or tell) your student to forward that information directly to you If you are providing financial support to them, make it part of the deal with them to tell you how they are doing Parents may obtain confidential records in one of three ways: Student shares the information with you Student signs a record authorization form (available on our Web site) Student is financially your dependent (proof of dependency required)

    19. Grades Your student will receive an email grade report approximately one week after the official quarter closing date. Fall Quarter grades will be emailed November 29. Your student may view grades online or request an email grade report from our web site.

Housing responsibilities. We received your Housing and Dining Contract along with your deposit. In return, we gave the student an initial room assignment. We also take care of configurations and Bobcat Cash administration, which I will talk about last. Please note that in your Facts for Parents card, Residence Halls close for Fall Quarter on November 21st at 6 pm even though the University does not officially close until the next day. TAKEN OUT We decided the configurations for the room and building and accommodate the student preferences.Housing responsibilities. Residence Hall Rooms For each student: Bed, closet, dresser space, desk and chair Computer and/or printer (based on student preference) 2 computers in quads Can bring your own computer (need Ethernet cable for wired access) Cable television service (need coaxial cable to wall) Telephone jack w/ local service (phone not provided) Microwave/refrigerator/freezer combination unit Only cooking device allowed, except for coffee/tea makers A/C in 70% of residence hall rooms Photos, floor plans & room dimensions on website Inside a student's room, he or she will have a bed, closet and dresser space. When first moving in please only bring what you need; make it easy on yourself during that time. Students can always bring more back with them from breaks after they get a sense of the space they have. Students will have a computer and printer based on their technology preference. You can bring your own computer. Wired Internet access is suggested in the residence halls, so just bring Ethernet cables for extra computers; hubs are already provided for you to connect to. Wireless service is also available. Free cable service is available, you just need a television and a coaxial cable. Make sure to get in touch with roommates beforehand so you don't end up with four televisions in a room. Free local telephone service is available, just bring a phone. If the student is using a calling card for long distance, its fine to continue using it. You can find more information about the Universitys long distance plan and more computer information at the Tech 101 session later today. The student is provided a microwave/refrigerator/freezer unit; do not bring your own. The microwave is the only cooking device we allow in the room, so don't bring a George Foreman Grill or a toaster; they will get confiscated. However, we do understand the need for coffee makers and tea makers, so those are allowed. 70% of the campus is air conditioned. Dining halls also operate on a cash basis. $75 is the max that students can spend per day and the most they will be liable for if the card is lost and OUPD is contacted in a timely manner. Bobcat Cash carries over quarter to quarter as long as the student remains registered. You can replenish Bobcat Cash with a check or online the same way you can make tuition and other fee payments.
