ANNUAL EDUCATIONAL AND OCCUPATIONAL OBJECTIVES FOR STUDENTS ENROLLED IN APDE-APPROVED CAREER AND TECHNICAL EDUCATION (CTE) PROGRAM Career Objective Form • Required by Chapter 4 Section 4.31 • Completed every year by each student • CIP code & Job Title included • Occupational title identified • Long-range goals identified • Must be signed and in student file • Student & teacher maintain a copy

PERFORMANCE ASSESSMENT • Performance Evaluation -Culminating activity to determine students competence in completing a task according to the pre-established standard. 1. Students are measured against a criterion (product or process) 2. Students should practice before evaluation 3. Teacher evaluates for final sign-off of task
TASK TRACKING • Teacher’s Responsibility • Task Check-off sheets • Convert to grades • Follows career objective • Filed each year in student file • Task tracking continues during co-op • Basis for awarding skills certificate(s) *See IEP Input Sheet
PERFORMANCE ASSESSMENT LOGS • Matches career objective • Student’s tool for tracking • Student’s responsibility for updating teacher/supervisor sign-off • Mandatory for school to career experience
