  2. 2009-2010 Right to Know/Hazard Communication and Blood Borne Pathogen Refresher Training Employee Instructions Employees will access the Questar III BOCES Health and Safety Services Right to Know and Bloodborne Pathogen refresher training and quiz through the following website- http://hs.questar.org/district All employees must complete the Right to Know/Hazardous Communication refresher training module. If you are in a targeted title under the district’s Exposure Control Plan, you must also complete the Bloodborne Pathogen training module (All Teachers and Teaching Assistants). Before beginning, you will need to get your employee number. It is on your paycheck stub or contact Nicole Lindeman via email or ext. 2611.

Log In-Employee will log in using their first initial and last name (lowercase). • Password-Employee will use their 4 digit payroll ID number, click on log in. • Employee must click the link at the bottom of the page. • Review the Right to Know module and complete the quiz. You will need a minimum score of 80%. For passing scores, a certificate of completion will be immediately generated. A certificate will also be emailed to you. Does the generated certificate need to be printed and submitted? No, it does not. • Employees covered under the Exposure Control Plan must complete the Bloodborne Pathogen training module and quiz. If after completion of the Right to Know module the site does not take you to the Bloodborne Pathogens module, please go back to the online training home page and select Bloodborne Pathogens at the top of the page. Review the Bloodborne Pathogens module and complete the quiz. You will need a minimum score of 80%. • Copies of the certificates of completion must be forwarded to your supervisor for proof of completion. The district designee will also monitor the training website for employee participation.

  4. All training must be completed during normal work hours so that the District Health and Safety Designee (Lisa Overocker or Neil Hailey) can be available to answer any questions that may arise during completion. You can also stop working on line and contact the designee the next day.
