Fatal blunder • Patsy Bryan, 39, was unlawfully killed when an anaesthetist mistakenly put an oxygen tube into her stomach during a routine operation, a jury at Leeds Coroner’s Court ruled. The mother of three died a day after the procedure at Leeds General Infirmary in 2002. Her family plan to press for criminal charges. Doctor cleared • A consultant anaesthetist who injected air into a baby’s bloodstream was cleared of manslaughter. A jury at Swansea Crown Court decided that the mistake did not constitute gross negligence. Dr Robert Falconer, 45, who wept at the verdict, may still face disciplinary action.
