Earlier Identification & Earlier Intervention More Effective & Successful Transitions 16+Learning Choices aims to: • Have a strong focus on earlier identification of need(s) and support(s) required • Facilitate effective transitions to positive and sustained destinations • Prioritise intervention for those most at risk of moving on to a negative post-school destination

  4. The Identification Process • 16 year-olds have typically spent 11 years in education • Schools have detailed knowledge of their needs and support requirements • Where this is lacking, partner agencies often hold key knowledge / relationships • Identification does / should start as early as possible

  5. 16+LC School networks • Clear in-school process to highlight & prioritise those most at risk of a negative post-school destination • Partnership working to enhance contextual assessment • Partnership working to identify most appropriate and effective interventions

Risk Identification Guide 16+LC Schools network – identified a gap/need for framework to apply consistency and robustness to decision making Aims: To facilitate the identification of those most in need of support to make an effective transition to post-school activity To support multi-agency problem solving and action planning for the most in need / disengaged population

  8. How good are we at predicting? • Data suggests we are ‘ok’ BUT far from perfect (Chronically disengaged, Sleepy NEET) • So...opportunity to learn from those we miss to inform and improve our processes
