-An update on research and treatment Achilles TendinopathyWhat is Causing the pain? Tobias Croner & Grant Watson
Introduction - MDT for the Extremities • Mechanical syndromes commonly diagnosed in spinal problems • Relatively new concept when applied to non-spinal musculoskeletal patients • How many fit the same symptomatic & mechanical responses?
Derangement • A disturbance in the normal resting position of the affected joint surfaces • Symptomatic and mechanical responses to loading strategies • Decrease, Better / Increase, Worse • Increased range and quality of motion and function • Remain better
Articular Intermittent End-range pain only Consistent Limited ROM No rapid changes Contractile Intermittent Pain with activity Resisted test positive Consistent Dysfunction
Survey of mechanical syndromes MII teaching faculty survey -15 consecutive patients 57 responses - 856 patients of who 242 = non-spinal (28%) Kevin Turner (UK faculty) audited own patients in UK 280 extremity patients through 2002 - 2003
Patients (N = 522) Mixed – acute to chronic Mostly working Smaller groups children, off work, retired Mean age 40
