Our Successes • Both units: • Significant improvements in process measures (screening for retinopathy, nephropathy, foot care, Self Management documentation) • Integration of learners into care models • Scheduled team meetings including case reviews allow increased coordination of care • Standard patient education materials and strategies for documentation identified (e.g., DM flowsheet)

Our Successes • MFP • Increased capacity and comfort of our clinicians with insulin starts and adjustments • Outcome Measure: • Significant improvement in patients at A1c targets • Stonechurch • Group Medical Visits • Endless possibilities! • Self-management paradigm shift • The Prepared Patient

  4. Key Learnings • Clinician awareness and buy-in central for success • Embrace group visits! • Consistent use of EMR by providers needed for clean data • Sustainability strategies with frequently changing team members (new learners) requires creativity
