Anti-ageing: The Hebe effect Age-ing is an inevitable biological physiological process. The desire to have a youthful andenergetic look, despite increasing age, is uniform across all age groups. Anti-ageing measures are divided into two categories: 1. MEDICAL INTERVENTIONS: Medical measures include topical creams and when usedcorrectly, decreases signs of ageing on the skin. Medical Measures include: a. Sunscreens - This is one of the simplest and most effective measures to prevent againstUV damage to the skin. It can be divided into physical and chemical sunscreen. Sunscreensblock and reflect UVB and UVA light that in turn prevents skin damage due to sun exposure. b. Antioxidants - The most commonly used antioxidant is glutathione followed by alfa-lipoicacid. Antioxidants cause increased collagen production that in turn increases the skinelasticity thereby providing tightness to the skin. c. Vitamin A and Vitamin C derivatives - Increases glycosaminoglycan component in thecollagen. It decreases the collagen degradation after UV exposure. Vit A is an importantcontent of anti-ageing creams. The net result is decreased pigmentation and wrinkles. Vit Cincreases collagen production in the skin that in turn increases the elasticity of thephotodamaged skin.
d. Organic / Natural compounds - The various commonly used compounds are green tea(anti-inflammatory), lemon and lavender oil (decreased oxidative stress), and chlorella(increased vascular endothelial growth factor). Ginkgo biloba, aloe vera and cucumber areother natural compounds that have anti-ageing properties. e. Peptides - Pal-KTTS and tripeptide copper complex increase collagen synthesis anddecreases sign of skin-ageing by causing skin tightening. f. Hormones - Estrogen containing creams are used for anti-aging and they act by increasingthe thickness, increasing hydration of the skin, and decreasing trans epithelial water loss(TEWL). The end result is natural elasticity to the skin. However, estrogen creams are mosteffective in post-menopausal women. 2. PROCEDURAL INTERVENTIONS: Procedural measures are more invasive but the effectis instant compared to medical measures. Procedural Measures include: a. Chemical Peels - Effective for treatment of signs of skin ageing and photo damage to theskin. It acts by causing coagulation of protein that leads to exfoliation of epidermis andsubsequent re-epitheliazation. Commonly used peels are Glycolic acid, Salicylic acid, Pyruvicacid and Jessner peel. Side effects observed are erythema / redness and irritation. b. Ablative LASER Resurfacing - These act by increasing transforming growth factor (TGF),tumour necrosis factor (TNF) and Interleukin (IL)-1.The net result of these is increasedcollagen synthesis and skin tightening. Commonly used ablative LASER’s are CO2 andEr:YAG LASER. c. Non - ablative LASER Resurfacing - These LASERs act by generating heat, which causes collagen damage and subsequent re-epithelization. They also act on microvasculatureand hence, neo-collagenosis. Commonly used LASERs are Nd:YAG, IPL and Light emittingdiode (LED). d. Microdermabrasion - Non-invasive and non-surgical procedure that is commonly used forskin rejuvenation. Inert crystals like Aluminium oxide and Diamond are used. Crystalsprojected through vacuum pumps onto the skin causes exfoliation of epidermis. Side effectsmaybe slight redness and irritation of the skin, which is transient. e. BOTOX - Injection of BOTOX (Botulinum Toxin) causes spasticity (tightening) of muscle atthe site of skin wrinkles and sagging (crow’s feet, frown lines on forehead). f. Dermal Fillers - As the name suggests, filling in the areas of decreased collagen;nasolabial (nose-cheek junction), below eye, angle of mouth lines. Dermal fillers aretemporary, semi-permanent and permanent. g. Lipotransfer - It’s a method of fat transfer and is primarily used for facial contouring. In thismethod, pre-adipocytes containing adipos derived stem cells is injected into the affected site.It acts by increasing the vascularity and fibroblast growth factor.
