National Primary Care Direction • More proactive care of Long Term Conditions • Need to manage acute demand differently • Aging population being felt everywhere • Aged residential care models evolving • International workforce shortages • More diversification of roles, new roles eg: Nurse Practitioner, Healthcare Assistant, Physician Assistant • More primary care services closer to home • Better, Sooner, More Convenient • Fewer complex, high tech services in as many sites across NZ
What’s happening with PHOs? • Government direction has been to merge PHOs • MRG Report suggested optimal size around 500,000 patients? • Merger of Kapiti, Tumai and Capital PHOs to form Compass Primary Health Care Network • New PHO is a Charitable Trust • Gordon Strachan is Deputy Chair of new Board • Local Primary Care Groups will be established to maintain local participation • All Kapiti local staff and the local office remain

What’s up with Kapiti General Practices? • Coastal Medical Rooms: BAU, No new enrolment currently • Team Medical: Dr Roland Lotsu from the UK locuming for 9 months from October. Not enrolling • Dr Chris Lane: not taking new enrolments except where urgent • Dr Paul Norton: BAU • Dr Bernard Cammack and Dr Ann Evans: BAU
GP Waiting List • Phone 0800 555 657 • Prioritised placement • Elderly, children and those who need regular care first • 226 people are currently on Waiting List: • All of the people currently on the list are waiting to be enrolled at their preferred practice. • For some of these people Hora Te Pai is their preferred practice. Hora Te Pai is a doctor short and will not be taking any new enrolments until this position is filled. • The Waiting List Co-ordinator is able to place anyone urgently requiring a GP immediately.
What’s happening at Hora te Pai? • Business as usual from service delivery perspective • Iwi, DHB and PHO unanimously committed to supporting the service • PHO Management support • New Service Manager Jeannine Stairmand and Clinical Leader Catherine Skelsey) • A new Charitable Trust to create some business separation from other activity • A Call for people interested in being appointed to Board, out very soon
Sharing Health Information • The most important ingredient in good decisions is having good information • Health Information across providers not accessible currently and needs to be …will save patients having to repeat it all over and over again • Manage my Health being trialled soon in the Wairarapa and in this area in the New Year • Will enable greater patient and provider access to information • Audit trail transparent to the patient
What is Urgent Community Care (UCC)? • UCC = Urgent Community Care • Very successful Pilot • Paramedics working differently • Reduced transport to hospital as support at home and liaise with other local providers
What about After Hours? • Team Medical • GP Roster • Waikanae • UCC: 24/7 • National and international trend to centralise after 10.00 pm • Few patients seen after that time who are not quite ill …transport to best care • We need to invest workforce resources wisely
