It WILL ask: • Name, sex, age, and date of birth • Race and ethnicity • Household relationship • Own or rent your home • It will NOT ask for bank account information, salary or income, citizenship or immigration status, and will never ask for your Social Security number. • Fewer questions than a credit card application!
