History • For grant year 2017-2018 • the county and the SEP (San Juan Basin Public Health) coordinated to streamline LTC applications/assessments using G-Suite • a finalized written procedure was implemented and in use to date Seeing the success of using G-Suite with the SEP, the county used grant year 2018-2019 to incorporate the Community-Centered Board (Community Connections Inc, CCI).

Goals • Streamline communication through a secure shared database • Increase transparency and accountability between agencies • Decrease the time it takes for an application or redetermination to be approved or denied • Improve staff morale because of more positive working relationships • Expand the sharing of information in G-Suite to include CCI

  5. Strategies • Use G Suite, a Google product that provides a secure common space to share documents • Explore the Network of Care through our regional No Wrong Door grant • Dedicate 1.5 staff members to this project

  6. Strategies Continued • Develop an internal DHS spreadsheet to track applications and redeterminations • Analyze each missed timeline to determine why it was missed

  7. Challenges • Unexpected staff turnover was a barrier but also a learning opportunity • Due to the smaller size of our CCB, referrals and communications were more sporadic, leading to an increase in questions and errors

  8. Lessons Learned • G Suite did everything we needed it to do and is affordable however, the new DSS1 form required by HCPF are not G-Suite compatible • Data reports available through HCPF do not track the timelines we need

  9. Next Steps • As of April 8, 2019, all CCI referrals/communications are being done in G-Suite. However, this ‘step’ has yet to really take-off • Finalize written procedure by June 30, 2019 • Work with HCPF on improving data reports
