9. State HIE Cooperative Agreements GrantsTemplate Data Purpose: To support planning and implementation of HIE. Requires states to: • Identify state level authority for HIT planning and implementation, • Appoint a agency level HIT coordinator, • Perform an assessment and develop and implement strategic and operational plans for HIE infrastructure, • Develop state level directories and enable state wide technology platform for HIEs, • Develop HIE business and operational processes, • Remove barriers to health information exchange and interoperability between electronic health record systems. For meeting those requirements states will be eligible to receive $4.0 to $40.0 million in federal support over the next four years.

Medicaid EHR Incentive Program • Requires Meaningful use of certified EHR system • Hospital EHR incentive program participants • Acute Hospitals • Children Hospitals • Eligible Non Hospital Based Provider EHR incentiveincludes: • Physicians • Nurse Practitioners • Certified Nurse Midwife • Physician Assistants (practicing in rural clinic or FQHC) • Dentist • Community Health Centers and Rural Clinics • Providers attestation and validation process: Medicaid will be determine provider eligibility and EHR eligible incentive payment amount • All processes must be auditable and transparent

11. StateNet Eligibility and and Website Policy • StateNet Liaisons (or designee) responsible for uploading documents to StateNet Webpages and any changes to documents. A password and log-in will be provided. • Each State webpage will include a template for inputting data. • Other State CIOs have access to webpages on read-only basis and may suggest changes by communicating with CIO State Liaison and other CIOs via in-state list-serves.

StateNet Eligibility and and Website Policy • CHIME members will register for their state’s list-serve through CHIME list-serves. • Any non-CHIME member CIO requesting participation in a particular state list-serve and website must register and be approved by the State Liaison. • Only CHIME members and registered non-CHIME members will have access to contact information for CHIME member State Liaison and other participants.
