In the Future • Healthwatch Doncaster is placed at centre of new Health proposals • Integrated Health and Social care services will need integrated information, support and advice services –Healthwatch Doncaster • An expectation on Councils to work with service users, their families and carers in designing services – through Healthwatch Doncaster
Capable Communities and Active Citizens We will continue to transform services in Doncaster – using the framework of the Government’s vision for Social care Prevention , Personalisation , Productivity Protection , Partnership , Plurality , People We will now look at the range of activity is planned for this transformation. Many of these projects will need input from Healthwatch Doncaster
Prevention • Developing a range of primary prevention support including volunteering, transport schemes, housing and befriending schemes • Develop an approach to identify, signpost and monitor those people at risk of losing their independence • Further improving access to equipment and adaptations • Further increasing the provision of telecare equipment • Linking up telecare and telehealth • Launching a universal non-assessed service • Providing telecare to people as a preventative service • Further developing community based intermediate care services • Improving arrangements for discharging people from hospital - Integrated discharge team & Positive Step assessment centre
Personalisation • Making sure our care management model is fit for the future • Increasing the take up of personal budgets and direct payments • Developing a more responsive and flexible service for carers • Developing employment opportunities for carers and people whose circumstances make them vulnerable
Productivity • Developing a quality assurance framework and publish how our services perform in a Local Account • Working with service users and carers to ‘co-produce’ policies and strategies • Improving access to information, advice and guidance on services in Doncaster for all service users including the development of the eMarket place – Connect for support • Working with Doncaster LINk to develop services and to set up the new service ‘Healthwatch Doncaster’ • Developing the provision of advocacy support
Protection • Working to improve the feelings of safety for those who use care services • Working with communities to reduce hate crime • Implementing the Safeguarding Adults strategy • Ensuring people placed out of area are in receipt of the most appropriate services
Partnership • Setting up the Health and Wellbeing Board, making sure that local people can influence the planning of local health and social care services.
Plurality • Reviewing day services • Reviewing the provision of in-house residential care • Ensuring an appropriate balance of provision in Doncaster between in-house and independent sector • Developing extra care and suitable housing options
People • Undertaking work to fully understand the local workforce • Completing and implement a local workforce strategy
