History of IM4Q • Planning Advisory Committee for ODP in 1997 developed Multi-Year Plan. • The aim of the Plan was to determine how people being served and their families feel about the services they receive. • The IM4Q is a data resource to the Administrative Entity and Providers for the Quality Improvement Plan. • ODP is using IM4Q data to respond to CMS
Intent and Purpose of IM4Q • To Collect independent data from people who receive services from ODP and their family • To ensure quality of life and satisfaction with services utilizing the Considerations Closing the Loop Process • To provide data to stakeholders of the ODP system • To ensure health of safety of people interviewed utilizing a major concerns policy

Time line January- Second round of Regional Meetings August/ September Sample lists are received by the AE. Training is conducted for monitoring teams March is the deadline to request additional samples is need April- Third round of regional meetings October- HCSIS usually opens to accept data entry First Round of Regional Meetings June all Data entry must be completed no later than the 30th Last Year Odessa did not open until December/ January July- annual Statewide training

WHAT CAN WE DO WITH THE IM4Q HCSIS DATA • Data is collected and analyzed by consideration themes, by participants, by interviewers, and SCO’s • IM4Q considerations for each participant can be printed directly from HCSIS • The data can illustrate patterns and trends • Data can be used by Quality Improvement Committees to facilitate organizational change • Aggregate data (PDF format) can be viewed on AE, State, and National levels
Nuts and BoltsWhat’s Important from SC Perspective? SC’s are responsible to complete a designated amount of pre-surveys distributed yearly by the Administrative Entity. These pre-surveys should be completed thoroughly to ensure that all information is correct. Check HCSIS alerts daily! You can bet once you’ve completed your IM4Q pre-surveys that the IM4Q teams will begin to schedule and facilitate meetings with the individuals you serve. Checking alerts is important. It is best practice to enter responses within 30 days of the local program entering it.
Nuts and BoltsWhat’s Important from SC Perspective? Respond to IM4Q considerations in HCSIS adequately and promptly. Ensure the “loop is closed”. There’s no loose ends that must be worked on.
Nuts and BoltsFinding Your Considerations in HCSIS Use your HCSIS credentials to log in to HCSIS. Once you’re logged in you can follow the menu path to find your considerations. M4Q à IM4Qà CONSIDERATIONS Once you reach the considerations screen, type in your consumers last and first name, then you will see all of his/her information. Click on your consumer’s name, and then you will see a list of all of his/her IM4Q considerations. Click on the hyperlink under “consideration ID” that will take you into the considerations box. Click the “select” bubble next to the consideration, and then click ‘edit” to begin working on his/her IM4Q in the box that says AE/SCO response.
Nuts and BoltsHow to Write a Thorough HCSIS Response A great and thorough response has these characteristics….. Has excellent details. Always be as specific as possible. Include dates, names, the entire plan to resolve the issue etc. The response shows a willingness by the SC to follow-up and “close the loop” The response is readable. Double check your writing. Check for proper grammar and spelling errors.
Consideration #1 Consideration Theme: Request for information Consideration: Mattie’s mother mentioned in the family survey she would like information on guardianship. Response: THE SC CALLED MATTIE'S MOTHER ON 7/10/2019 TO DISCUSS GUARDIANSHIP FOR MATTIE AND DISCUSSED THE NECESSARY STEPS NEEDED TO ESTABLISH GUARDIANSHIP. THE SC REMINDED MATTIE'S MOTHER ABOUT GOING TO COURT, HAVING MATTIE ESTABLISHED AS INCOMPETENT, AND THE ASSOCIATED LEGAL FEES THAT ARE THE RESPONSIBILITY OF THE INDIVIDUAL PETITIONING FOR GUARDIANSHIP. MATTIE'S MOTHER EXPRESSED HER CONCERN THAT SHE BELIEVES THAT MATTIE IS NOT ABLE TO MAKE DECISIONS FOR HERSELF AND THE SC ADDRESSED HER CONCERNS. THE SC CHECKED HCSIS SERVICE NOTES TO SEE WHEN THE LAST TIME THE TEAM SPOKE ABOUT GUARDIANSHIP. THE SC ASKED MATTIE'S MOTHER IF SHE WOULD LIKE THE INFORMATION SHE RECEIVED LAST YEAR ABOUT GUARDIANSHIP AGAIN AND SHE REPLIED THAT SHE WOULD. ON 7/10/2019,THE SC LOCATED THE INFORMATION AGAIN AND MAILED TO HER FOR HER REVIEW.ALSO ON THIS DATE, THE SC LOCATED AND REVIEWED ADDITIONAL DOCUMENTATION RELATED TO GUARDIANSHIP AND ALTERNATIVE OPTIONS. THE SC RESEARCHED THE PROCESS CONNECTED TO ESTABLISHING GUARDIANSHIP AS WELL AS REVIEWING DOCUMENTATION FOR ESTABLISHING AN ADVANCED HEALTH CARE DIRECTIVE, FINANCIAL GUARDIANSHIP, REPRESENTATIVE PAYEE, AND THE LIMITATIONS OF GUARDIANSHIP/DIRECTIVES. THE SC FOLLOWED UP WITH MATTIE'S MOTHER DURING A MEETING FACILITATED ON 7/15/2019, TO ANSWER HER QUESTIONS AND REVIEW THE STEPS NECESSARY TO ESTABLISH GUARDIANSHIP AS WELL AS PROVIDING HER MORE INFORMATION ABOUT ALTERNATIVE METHODS TO SUPPORT MATTIE THAT ARE LESS RESTRICTIVE. THE SC ALSO SHARED DOCUMENTATION, INCLUDING LOW COST LEGAL RESOURCES IN PHILADELPHIA AND PRESENTED IT TO MATTIE AND HER MOTHER ON 7/15/2019.
The Loop is Closed The loop is closed because the IM4Q consideration only made mention that MATTIE'S mom would like more information regarding guardianship. The Supports Coordinator located guardianship resources to assist Mattie’s mother in the future. Also, the Supports Coordinator did an excellent job providing details in the response.
Nuts and BoltsWhat’s Important to Remember from the SC Perspective? Always complete your IM4Q consideration responses honestly and in its entirety. Check your HCSIS alerts often to check if you have IM4Q considerations that must be addressed. Be timely in writing your responses (within 30 days) Always make sure that the individuals we are serving are satisfied with the outcome. It’s their life,let them live it well! IM4Q responses may require follow-up from the AE, don’t forget to do the follow-up! It’s imperative in closing the loop.
