Changes to improve data quality • Monthly blocks with date for every blood test and measurement STOP quarterly block • All drugs and prescriptions , start and stop dates submit quarterly list • Comorbidity with dates of diagnosis – 1 block cumulative data • Haemodialysis block – data for every session
