Lifestyle Management – Telephonic Coaching What to expect next: If risks are identified through the Health Risk Assessment you may receive a phone call from one of the Healthways Lifestyle Management Coaches. Program is confidential, free and voluntary. Risks addressed will include physical inactivity, weight management, tobacco cessation, nutrition, hypertension, high cholesterol, etc. Coaches are trained in behavior change and are from backgrounds including nursing, exercise science, nutrition and health education. Flexible Spending Accounts (FSA) Save on Health and Dependent Care • Use pre-tax dollars for every day expenses for yourself, spouse, qualifying child, or qualifying relative. • Cover health care needs, dependent care, or both • For further explanation on who is considered to be a qualified dependent, please visit the Wage Works website at www.wageworks.com. • You do not have to be enrolled in a City of Norfolk medical plan to enroll. • Employees who are already enrolled in an FSA account through their spouse’s employer can still enroll. However, it is solely the participant’s responsibility to follow IRS regulations and guidelines.

Prepare for a Secure Future • If you become sick or injured outside of work, • We have a plan for you. • First: Accumulated leave – receive 100% of your earnings • Second: Short Term Disability Program - Sick Leave Bank – receive 100% of your earnings • Third: Long Term Disability Insurance – The Standard Insurance – receive 67% of your earnings
Norfolk Sick Leave Bank • To become a participant you must: • Enroll within the first 30 days of employment with the City, or • Have at least ten (10) days of Sick Leave saved to enroll during open enrollment. • Policy and Procedures: • Applications for “withdrawal” from the Bank should be submitted as soon as you know that you will be on an extended absence from work for a non-occupational medical condition. • You must be absent from work for at least thirty (30) workdays AND exhaust all accrued leave before becoming eligible to withdraw from the Sick Leave Bank. • You must maintain contact with your department and the HR Sick Leave Bank Coordinator and submit updated documents in order to maintain eligibility. • The maximum amount you can withdraw from the Sick Leave Bank is 100 days per disability or per 12 month period. Once your total “lifetime” withdrawal reaches 250 days you are no longer eligible to participate. • The Sick Leave Bank Policy is detailed in the Administrative Policies Manual on the HR intranet. http://intranet/hr/Policies/admin/admin_policies.pdf
Long Term Disability (LTD) Insurance • LTD insurance kicks in 90 days from the time of the event • You will receive 67% of your earnings after all other forms of leave has been exhausted. • Benefit payments are offset by other sources of income you eligible for (i.e. Worker’s Compensation and Social Security) • LTD payments will not be less than $100 or more than $10,000 per month • Benefit payments are not subject to Federal or State income taxes. • Eligible to receive benefits until age 65if disability occurs before age 62. If disability occurs at age 62 or older, benefits duration is based on the age when disability occurred.
