First In-Home Consultation • Answer questions from information session. • Discuss the FE/Approval Process • Review Self-Assessment and Reason for Fostering questions • Tour the Home • Provide Evaluation Paperwork Packet (EPP)
Second In-Home Consultation • Discuss any questions/concerns • Assess the family’s progress in pre-service training and completing required paperwork. • Discuss contents of their Learning Journals • Begin interviewing family members • Review results from questionnaire, assessments including the Casey Family Assessment • Determine progress made on completing Physical/Medical Information, Drug Screen, Fingerprints, CPR/1st Aid
Third In-Home Consultation • Complete interview process • Discuss the family’s decision to foster or adopt; • Discuss the family’s preference of a child(ren) to be placed in their home and complete Placement Preference form. • Discuss potential resources that the family may need; • Discuss the next steps • Discuss the family’s continuing education plan. • Collect any additional paperwork

CHAP • Available Time Scale-ATS • Alcohol Use Disorders Identification Test-AUDIT • Foster Parent Role Performance Scale-FPRP • Receptivity to Birth Family Connections Scale-RBFC • Reasons for Fostering Checklist-RFC • Social Readjustment Rating Scale-SRRS • Cultural Receptivity Scale-CRFS
Process for Foster and Adoptive Parents • Each parent is required to independently complete each of the seven CHAP scales • Each parent independently enters a score on the cells next to each item-each scale has a different scoring system • Submit completed CHAP Scales at required intervals to case manager completing Family Evaluation
Processfor Case Managers • Open the CHAP Excel Scoring Program (2 MB).Once downloaded, click "Enable Macros" and then click "Read Only" • Complete the Background Information worksheet for each family • Enter data from the submitted forms onto the CHAP Excel Scoring Program in each of the respective worksheets
