Evidence of Success for the Class of 2015 • 2015-2017 • One county had reduction in ED and inpatient hospitalizations with diagnosis including HIV • One county had reduction in ED and inpatient hospitalizations with diagnosis of Hepatitis C • One county had reductions in ED and inpatient hospitalizations with co-occurring substance use diagnosis and endocarditis • Same county had reductions in ED and inpatient hospitalizations with co-occurring substance use diagnosis and Hepatitis C
SEP Class of 2016 Thirteen counties: Boyd Carter Clark Franklin Grant Harrison Jessamine Knox Mason Mercer Perry Pike Warren
Evidence of Success for the Class of 2016 • 2016-2017 • 8 of 13 counties had reduction in ED and inpatient visits with HIV diagnosis • 5 of 13 counties had reduction in ED and inpatient visits in both endocarditis and Hepatitis C • In visits with infectious disease cases co-occurring with substance use . . . • 5 saw reductions in SUD and endocarditis • 4 saw reductions in SUD and HIV • 3 saw reductions in SUD and Hepatitis C
Classes of 2017 and 2018 • 2017 - 20 counties • Data will be available soon for one year change analysis • 2018 - 10 counties • Data will be available in spring 2020
