The National Demonstration Project(NDP) www.transformed.com/ndp 24-month Project Project rigorously pilot tested the TransforMED patient-centered model-of-care in 36 family medicine practices from across the United States. The NDP pilot project concluded in June of 2008.
Initial NDP Learnings • Transforming to a patient-centered medical home (PCMH) isn't easy…requires change throughout entire practice (Leadership, Teamwork, Communication) • Medical practices can be lonely places: physicians and staff often feel isolated. (peer-to-peer learning) • Do it Yourself Attitude (DIY) • Technology isn’t plug and play but critical to medical home transformation • NDP Outcomes (No external sources of revenue) • 10.49% increase in practice revenue • 14% increase in physician salaries

Match Results Impressive AAFP News Now Match Results: 2010 Fill Rate for Family Medicine Highest Ever By Barbara Bein3/18/2010 2,404 family medicine positions were filled out of 2,630 positions offered, for a fill rate of 91.4 percent, the highest percentage of family medicine positions ever filled in the NRMP.
P4 Demonstration Initiative www.transformed.com/p4 For the first time, all 14 residencies in the Preparing the Personal Physician for Practice, or P4, Demonstration Initiative filled their open positions during the Match.
