WVHIN • How is the WVHIN achieving its purpose? • Enabling the secure ELECTRONIC exchange of patient health information through: • WVDirect Secure Messaging • Health Information Exchange (HIE) • Why are we working to achieve this purpose? • Today patient information is stored in silos and access to this information is not timely, therefore providers must make decisions about a patient’s health without the full picture • Facilitate accomplishment of Meaningful Use • Create efficiencies and cost savings within the healthcare system

Improve your quality initiatives • Healthcare Organizations are faced with multiple quality initiatives designed to improve care (i.e. NCQA, Meaningful Use, PQRI, PCMH) • Two key factors in meeting quality standards are reducing unnecessary ER visits and hospital readmissions • Communicating with outside institutions to improve these quality measures is not efficient and often nonexistent. The “transitions of care” into and out of our offices and hospitals can be better communicated with WVDirect
