2014 Open Enrollment Highlights • HealthPort continuously assesses benefits costs in order to determine any necessary program changes while striving to provide a comprehensive benefits package to all eligible associates. Here are the 2014 highlights: • Transamerica & Key Benefit Administrators will be replacing United Healthcare as the provider of the limited medical plan. • The medical plans will now cover preventive visits at 100% • The Standard will continue to provide the dental and vision insurance with no changes to benefits or price. • We will be changing Health and Dependent Care Spending Account administrators to Benefit Resources Inc. • The critical illness, accident, and legal plans have been discontinued.
Considerations for 2014 Open Enrollment • 2014 Open Enrollment is taking place October 14th – 25th • Elections take effect January 1, 2014. Pre-Tax elections cannot be changed during the year unless you have an IRS qualified event • You are now able to enroll in coverage via the state exchanges. Visit www.healthcare.gov for more information.
Enrollment Instructions • We are having an active enrollment. You have to make an election to continue coverage. If you take no action, we will assume you are waiving coverage. • Go to the UltiPro Homepage and select Myself > Open Enrollment • Select the current open enrollment session • Verify and add any new dependents • Follow the steps to enroll or decline in each benefit
Transamerica & Key Benefit Administrators Medical Plan Options
Transamerica & Key Benefits Administrators Health Plan • Two separate plans and ID cards, one ID number • Preventive Services card provided by Key Benefits Administrators • Preventive coverage only covered at 100% • Limited Hospital Indemnity Benefits provided by Transamerica • Provides reimbursements for sick visits, hospital stays, prescription drugs • Offered as a package, cannot be elected separately • Enrolling in this benefit allows you to avoid the individual tax penalty of Health Care Reform.
How are claims processed? • Give the appropriate ID card to your medical care provider • Benefits for preventive services are paid at 100% for network providers • If the service is not preventive, you will receive a check for your benefit amount.
Covered Preventive Services for Adults • Abdominal Aortic Aneurysm one-time screening for men of specified ages who have ever smoked • Alcohol Misuse screening and counseling • Aspirin use for men and women of certain ages • Blood Pressure screening for all adults • Cholesterol screening for adults of certain ages or at higher risk • Colorectal Cancer screening for adults over 50 • Depression screening for adults • Type 2 Diabetes screening for adults with high blood pressure • Diet counseling for adults at higher risk for chronic disease • HIV screening for all adults at higher risk • Immunization vaccines for adults--doses, recommended ages, and recommended populations vary: • Hepatitis A • Hepatitis B • Herpes Zoster • Human Papillomavirus • Influenza (Flu Shot) • Obesity screening and counseling for all adults • Sexually Transmitted Infection (STI) prevention counseling for adults at higher risk • Tobacco Use screening for all adults and cessation interventions for tobacco users • Syphilis screening for all adults at higher risk
22 Covered Preventive Services for Women • Anemia screening on a routine basis for pregnant women • Bacteriuria urinary tract or other infection screening for pregnant women • BRCA counseling about genetic testing for women at higher risk • Breast Cancer Mammography screenings every 1 to 2 years for women over 40 • Breast Cancer Chemoprevention counseling for women at higher risk • Breastfeeding comprehensive support and counseling from trained providers, as well as access to breastfeeding supplies, for pregnant and nursing women* • Cervical Cancer screening for sexually active women • Chlamydia Infection screening for younger women and other women at higher risk • Contraception: Food and Drug Administration-approved contraceptive methods, sterilization procedures, and patient education and counseling, not including abortifacient drugs* • Domestic and interpersonal violence screening and counseling for all women* • Folic Acid supplements for women who may become pregnant • Gestational diabetes screening for women 24 to 28 weeks pregnant and those at high risk of developing gestational diabetes*
22 Covered Preventive Services for Women • Gonorrhea screening for all women at higher risk • Hepatitis B screening for pregnant women at their first prenatal visit • Human Immunodeficiency Virus (HIV) screening and counseling for sexually active women* • Human Papillomavirus (HPV) DNA Test: high risk HPV DNA testing every three years for women with normal cytology results who are 30 or older* • Osteoporosis screening for women over age 60 depending on risk factors • Rh Incompatibility screening for all pregnant women and follow-up testing for women at higher risk • Tobacco Use screening and interventions for all women, and expanded counseling for pregnant tobacco users • Sexually Transmitted Infections (STI) counseling for sexually active women* • Syphilis screening for all pregnant women or other women at increased risk • Well-woman visits to obtain recommended preventive services*
Standard Dental Passive PPO Low Option New enrollee waiting periods apply, 6 months for Basic services, 12 months for major services
The Standard: Supplemental Life Insurance Supplemental Employee Life Supplemental Dependent Life Evidence of Insurability (EOI) will be required for Employee, spouse and dependents if you have not previously had coverage.
The Standard: Short Term Disability Evidence of Insurability (EOI) will be required during Open Enrollment if you don’t have current coverage. * Effective 1/1/14, benefit deductions will be taken out of disability checks
