Growing Concerns about LTC Culture • Hierarchical organisational structure • Predominance of acute, medical model of care • Unidirectional, provider-as-expert approaches • Program-centred rather than person-centred • Reliance on outcome measures irrelevant to effective care decisions • Exclusion of those directly involved in care and support in decision-making (i.e., persons with dementia, family members, front-line staff)
