Improvement as a Path to PCMH • 10 FQHC look-alike clinics • Clinic staff meetings every other week • Plan-Do-Study-Act (PDSA) cycle as a vehicle • Regular coaching • Goals: engage front-line staff, foster innovation, standardize and spread, PCMH
