  5. SUBJECTIVE : here is the patient with 67y male subjected with cough with expectoration since 20 days and fever since 20 days. SOAP

  6. OBJECTIVE SPUTUM : GRAM NEGATIVE BACILLI IS SEEN.

  7. Problem 1 :cough It occurs due to the presence of any exogenous substances and a endogenous stimuli where it expels the obstruction causing substances from respiratory tract problem 2 : fever Its occurs due to the presence of pyrogens which are the toxic substances released from the disease causing organisms Assessment

  8. problem 3 : type 2 DM It occurs due to the insulin resistance to the tissue and poor intake of glucose by skeletal muscles. Problem 4 :asthma Its a type 1 hypersensitivity reaction which is characterised by reversible airflow obstruction and bronchospasm.

  10. TYPE 2 DM: T. Glycomet :metformin 500 mg It acts by decreasing the hepatic glucose production and peripheral insulin sensitivity. Bronchial asthma. Inj.deriphyline Acts by Inhibiting of phosphodiasterace and antagonism of adenosine receptors,therby causing bronchodilation. Plan

  11. Syr.bruzedex :bromohexene hcl it act as an expectorant/mucolytic agent which expels out the mucus secreated. In.cefra :ceftrixone it exhibits bactericidal activity by the inhibition of bacterial wall synthesis by binding with cephalosporin binding proteins.

T.akurit 4:[ isoniazid ,rifampicin,pyrazemide,etambutanol ] • : Isoniazid :it acts by inhibition of mycolic acid synthesis and disruption of the cell wall in susceptible organism : Rifampin: a semisynthetic broad-spectrum bactericidal antibiotic, inhibits bacterial RNA synthesis by binding strongly to the beta subunit of DNA-dependent RNA polymerase, preventing attachment of the enzyme to DNA, and thus blocking initiation of RNA transcription [2 • Ethambutol :. It inhibits the synthesis of certain metabolites subsequently impairing cell metabolism leading to cell death.
