  1. Giving a talk in Chicago called “Simple Tools for Better Care, Chronic Disease Management through Patient Centered Collaborative Care. This phone conversation is a spin off of that material “The talk should actually be called “Deceptively Simple Tools for Better Care” as the tools we are discussing can be used very simply and effectively in patient interactions, but also offers the potential for a deeper analysis of your practice and how it affects patient outcomes. We’ll look at both levels today, briefly. Then during Q & A, we can also hone in on questions about the best way to apply these individual tools to your practice.
