AWC Health and Wellbeing Hub • Update from TIDB • Agreement and support for VCS Community Health navigators bid • Phase 2 now started – integration at Locality Level • Restructure of meetings within transform – development of ICSIG – needs A,W and C reps for that meeting – recruit. Plus for relevant work streams. My aim is to diversify and strengthen the role of VCS reps. • Building Health Partnerships update: Hub Development worker.

AWC Health and Wellbeing Hub • Health Navigators Bid. Their role is to: • Become the expert in each locality (AW and C) on what VCS services are represented in each area. • Build relationships and referral pathways with each of the VCS providers • Represent the VCS at each of the 8 MDTS (0.5 worker will be attached to each Locality) • Ensure that the patients discussed at the MDTs have their needs discussed and appropriate referrals are made to VCS providers
AWC Health and Wellbeing Hub • The navigators will act as single points of contact for referral in to MDTS therefore allowing the VCS the same access to MDTs as all Health Services. • Promote the appropriate use of the VCS within the MDTS • Ensure the development and maintenance within each locality of the Community Health Map areas are undertaken and that it remains relevant. • Identify any key gaps within the VCS in each locality and feed this back to the relevant commissioners
AWC Health and Wellbeing • The benefits of this project are multiple: • Improved patient experience and outcomes at MDTS • Improved knowledge of VCS by MDT Staff • Reduction in GP appointments needed through individuals accessing appropriate VCS support • Reduction in admission to AGH and Urgent Care pathways through individuals accessing appropriate VCS support • Ensuring the Community Health Maps are the key strategic directory for AWC and remain up to date and useful for all GPS. • Ensuring that all VCS providers are mapped and gaps identified.
