Tab1 – Project Summary • Project Information – At the top of page one you must provide general information about the project • Instructions – There are instructions that provide an overview for how to complete the form • Checklist of Forms and Attachments – The checklist serves as a reminder to complete the required sections and include all required materials with your submission.
Tab 1 – Project Summary • Building Information • Provide information about the building(s) including Building Type, Units and Commercial Space • Complete the required information in the Floor Area Breakdown
Tab 1 – Project Summary • Scope of Work • Depending on whether the project involves new construction or rehabilitation, complete the appropriate section.
Tab 1 – Project Summary • Quality of Vertical and Site Improvements • In the Quality of Work section, you must select one of six options that best describes the overall quality of work • The user must also provide a detailed explanation to justify the Quality Selection
Tab 1 – Project Summary • Special Development Conditions • Depending on the type of project, there may be special development conditions that will be included in the scope of work. Provide a detailed explanation and estimated costs associated with the special development conditions in the spaces provided in this section.
Tab 2 – Schedule of Values • This section of the form provides specific information about the project costs along with an itemization of the costs to develop the site • The construction costs must match the costs on the Rental Project Workbook along with the cost documentation provided with the AHP application.
Tab 3 – Construction Budget Summary • This section summarizes the itemization of costs that were entered in tab – 2 Schedule of Values and segments those costs into specific groups
Tab 3 – Construction Budget Summary • Required Signature - After the entire form has been completed and verified for accuracy, the project architect or a qualified third party must print and sign the signature page then scan the signature page as a PDF and submit it to the Bank along with the application
