Serious Loss/Root Cause Analysis • 35 incident/collisions completed • Focus on high exposure incidents/collisions • Recommended corrective actions identified in Best Practice report • What trends are we seeing • Repeat offenders • Policy enforcement inconsistencies • Collision preventability vs passenger injury assessments
