Innovative Team Structure • Joint team working across the two counties of Conwy and Denbighshire • Each team member has either a defined lead or support area • Ability to develop expertise within specific areas • Lead or Support areas are across both counties • Challenge to avoid duplication and encourage strong partnership working

  4. Conwy and Denbighshire Immunisation Working Group • Joint group with broad membership • “Virtual network” for help and support regarding immunisation • Use of specific expertise as required • Immunisation training sub group organises joint training of immunisers

Membership of the Group • Representatives from C and D Trust and both LHBs • Paediatric Consultant • LHB Pharmacy Rep • LHB Senior Nurse • Principal PH Practitioner • NPHS Specialist Nurse (immunisation) • LHB Practice Support Nurses • LHB Performance Management Rep • Senior Nurse Manager Young People’s Health Advisors • Regional NPHS Immunisation Lead • Chaired by Acting Local Public Health Director

Childhood Immunisation Action Plan • Purpose to encourage activities and good practice that promote vaccine uptake • Written in collaboration with partners • Stakeholder agreement of objectives and ratified at both Local Health Boards • Incorporates the learning from the Denbighshire Immunisation Project • Evidence Based Action Plan

Childhood Immunisation Action Plan • Vehicle for Implementing Welsh Health Circulars • Promotes regular joint discussion of local immunisation issues through updates • Actions for GPs Trusts LHBs • Accessible on NPHS website • Implements key messages of Core Module Strategies to Promote vaccine uptake

Childhood Immunisation GroupBenefits • Regular reporting against Action Plan encourages a sense of accountability • Raises profile of immunisation amongst wider audience • Provides a vehicle for building stronger relationships between agencies • Promotes best practice • Clarifies the role of individual stakeholders

Pilot Community Pharmacy Influenza scheme • Pharmacy staff identify patients who come into the pharmacy who would be in one of the groups for whom vaccination is recommended • Asked - had they received the flu vaccination • Answered ‘yes’ no further input required • Answered ‘no’ brief advice provided • Timescale - End October to End November • Head of Pharmacy & Medicines Mgmt, DLHB • Blackpool Primary Care Trust

  11. Results • 75% of those customers in receipt of prescriptions or over the counter drugs had received flu vaccination Of those who had not had the vaccination • 18% responded they would have the vaccination, 21% maybe

Conclusion • Community Pharmacies provide a positive contribution to improving public health • Community Pharmacies see a group of patients that may have missed or ignored campaign messages • Community Pharmacies see those within the at risk groups, who are otherwise healthy, collecting repeat prescriptions

  20. Denbighshire CHIGPurpose • To provide coordination of activity for Health Improvement • Coordinate the work set out in RTHI • Develop and monitor an action plan • Agree key objectives and indicators for Health improvement groups

  21. Denbighshire CHIG • Facilitate the sharing of knowledge and understanding of health improvement agenda • Promote the roles of individuals , organisations and partnerships within the Denbighshire wider network
