Forms Merged: Goal Setting and Mid Year Review Rational: In order to answer the questions in the mid-year review, teachers and administrators needed to go back to the goals form to review data from the fall. Mid Year Review now linked to Goals.
Form Revision and Merger: Announced Observation Evidence/Report. Teachers have the option to submit evidence in their observation form. It will be populated into the rubric. Note: Teachers will be able to see observation saved as a draft. Pre-Conference Form was eliminated because these questions have been added to the observation form.
Form Revision: • Announced, Unannounced and Walk-through forms. The administrator will now be able to type in the date of the observation. Rational: The date the forms are submitted may not occur the same day. Date specific
Form Revision: • Announced, Unannounced and Walk-through forms. A teacher will have the option of typing in comments prior to submitting any form. These comments will be populated into the summative rubric. Rational: Gives teachers the option to provide input to any forms. Comments Count!
Key Components: Revised Teacher APPR Forms • Merged the Professional Goal Setting Form and the mid year review form. All data will populate directly into the rubric. • Teacher and administrator can both contribute to the Announced Observation Form. The pre-conference form was eliminate because these questions are now located on the observation form. • The administrator will now be able to type in the date of observation. • Teachers can submit comments prior to acknowledging any form and this data will now be populated into the summative rubric.
