Key Components: Revised APPR Forms • Eliminated 3 optional forms (1A Self-Reflection, 2A Lesson Plan and 2D Cumulative Structured Review). • Teacher has the option to upload a lesson plan as an artifact, tagged to the rubric. • Professional Goal Setting Form will populate data into the rubric. • Teacher and administrator can both contribute to the Pre-Conference Form. The self-reflection form was eliminate because these questions are address in the pre-conference form and populate into the rubric.
Key Components: Revised APPR Forms • Announced Observation is now called the Announced Observation Evidence/Report. (Unannounced Observation = Unannounced Observation Evidence/Report. All of the standards/indicators have been added to the observation and walk-through forms eliminating the need for the cumulative structured review form.(Note it is not an expectation that you would collect evidence for each standard or indicator). • Mid Year Meeting Evidence/Report: To be completed by the teacher or together with the administrator. This form is for teachers who in the developing or ineffective range. It can also be used at the administrator’s or by a teacher’s request.
Forms Eliminated:1ASelf-Reflection, 2A Lesson Plan & 2D Cumulative Structured Review Rational: The questions in these forms were redundant and are embedded in other forms. Teachers can reflect in the preconference form. Teachers can upload a lesson plan using the artifact upload. This will ensure the lesson plan populates to the rubric.
Form Revision: • Professional Goal Setting Form will populate data into rubric. • Goals describe, results, impact or outcome on teacher practice or student learning. Goals should be specific, measureable, attainable, relevant/realistic and timely. • Goals are now tagged to the standards. Artifacts teachers submit during the year should be tied to their goals.
