3. User Name and Password Username: nsw_XXXXXXX (Region and school code combined)
Password: YYYYY (5 Digit number unique to your school)
The username and password will be provided by your regional contact.

You will be required to enter the Username and Password supplied to your school

5. Choosing surveys All schools have access to 10 surveys
4 primary surveys
4 secondary surveys
1 leadership survey
1 community engagement survey
6. Go to the My Survey section at the top of the page
Once you have clicked on this, the surveys available to your school will be displayed
7. There are 10 surveys for schools
Select the survey – eg Primary K-6 Student Survey - NUMERACY
Hit the Collect icon
8. On the next screen, click the Add New Collector
9. The next action is to determine how the information to collect the survey will be sent

Place hyperlink on school webpage
Type in the name for the link and click next step
10. Inviting others to the survey
11. You will need to copy the hyperlink (email or webpage)
You will need to check the settings to make sure that more than one person can answer the survey
12. When the survey link comes up, go into the settings
13. Ensure that, “Yes” is selected on the multiple responses
14. Once settings have been saved select the “Back to Summary” Tab
15. You are returned to this screen where you can copy the hyperlink to send out in an email.

Use a group list to send to all staff (executive and teachers) or all students in a Year, for example
17. How to send the email out to a student group using your portal webmail
18. A new message window appears. Select the To… button which will open up the address book
19. Select the student address book. School teachers will have the students in the school listed
20. To select a group of students for the email, hit the “Shift” button on the computer to select the first student and then hit the “Shift” button to end the selection. The names to be selected will be highlighted.
21. Select the To button on the bottom of the page and the students selected will appear as names in the To box.
22. When you return back to the message you will now see the names in the “To…” box
23. Add the subject to the email and paste the hyperlink generated by survey monkey. Type in any other comments, including the School Code.
24. The email has been sent here to the sender as an example for you to see.

26. Answering the survey
27. The hyperlink will take staff / students to the survey, where they can answer the questions
28. All questions must be answered - if any questions are blank This question requires an answer will appear
When completed click “Done”
29. Looking at the survey results
30. To analyse the results of the survey, select analyse results tab on top right
31. The Staff survey was updated1 minute ago and there are now 4 responses
