Patients with insufficient benefit from IVIG therapy may decide to undergo surgery. Day 28 C = maximal length of crossover. Crossover (C): Day 1C Day 5C Day 9C. Patients with insufficient benefit may decide whether they want to cross over to the other treatment arm or undergo surgery.
