In order to be eligible for MassHealth, Commonwealth Care, or the Health Safety Net, you must be a Massachusetts resident. The information reported to this office says that you live in the state or territory listed below: • If this letter is for an adult, you must send us proof that you live in Massachusetts. We accept the following types of documents to verify residency: • lease; • city or town real estate tax bill; • notarized rent receipt; • current utility bill(s); or • notarized affidavit supporting residency. • If this letter is for a child, please send proof that your child went to a Massachusetts school for this school year or send nursery-school or day-care records. • Please return your proof in the enclosed pre-addressed envelope. Please cut off and return the bottom section of this notice with your proof. • You must send this information within 45 days of the date of this notice. If you do not send the information, your benefits will end.
