  2. 2011 GRANT PROGRAM • THE SANDUSKY COUNTY COMMUNITIES FOUNDATION • Founded April of 1998. • Originally founded to establish a scholarship in the memory of Sara Barbour, the granddaughter of Bob Moyer. However, using foresight and a goal of one day having funds sufficient to support an independent local foundation, the board established a broad charter to allow for charitable purposes beyond scholarships. • The Columbus Foundation was hired in 1998 to provide guidance and administer the Foundation. Through 2007, the foundation had grown to nearly $1.4 million in assets and had 10 donor directed funds. • As the result of a $1.45 million gift, an independent 501(c)3 was established in 2008. This independence allows The Sandusky County Communities Foundation, Inc. to provide charitable donations at their discretion throughout Sandusky County.

  3. 2011 GRANT PROGRAM • THE SANDUSKY COUNTY COMMUNITIES FOUNDATION, INC. • Currently has nearly $ 2.8 Million in assets. • Is in the process of moving the funds currently managed by The Columbus Foundation back to Sandusky County to be managed locally. • Has established guidelines to facilitate an annual giving program, however the local board still maintains the ability to make community contributions outside of the established annual giving program. The awards being presented today represent the 2nd installment of this program.

  5. 2011 GRANT PROGRAM The Board of the Foundation would like to acknowledge the passing this past May of Board Member Gary Zajac. Gary provided the board with valuable input and insight from Woodville and the western part of Sandusky County. His knowledge and humor will be missed. In his honor, the Foundation has made a $1,000 contribution to Woodmore C.A.G.E., an organization that supports the students of Woodmore Schools.

  8. 2011 GRANT PROGRAM • The Grant Review Committee reviewed: • 22 Grant Applications. • Nearly $120,000 in grant requests. • As a result: • Thirteen grants will be awarded. • $ 55,648 in giving. • These funds will be used to support projects totaling nearly $103,000.

  10. 2011 GRANT PROGRAM AMERICAN RED CROSS, SANDUSKY RIVER CHAPTER • Project Title: Red Cross Ready Project • Grant Amount: $ 3,890.00 • Purpose: • Funding will provide for the acquisition of items to support disaster preparedness and response, including: • Laptop computer for portable communications. • Portable privacy screens. • Portable folding tables.

  11. 2011 GRANT PROGRAM AMERICAN RED CROSS, SANDUSKY RIVER CHAPTER • Project Title: Child Abuse Recognition and Prevention Upgrade • Grant Amount: $ 1,355.00 • Purpose: • Funding will support the purchase of materials and tools to be used in the training of child care providers in the recognition and prevention of child abuse, including: • DVD training videos. • A shaken baby simulator. • In 2010, the Red Cross trained 74 child care providers in the county.

  12. 2011 GRANT PROGRAM • BELLEVUE CITY SCHOOLS • Project Title: BELLEVUE REDMEN GOOD VIBRATIONS • Grant Amount: $ 1,766.85 • Purpose: • Complete the funding towards the purchase of a three-octave set of bells. • In 2010, the Foundation provided a grant of $2,500 towards the purchase. The remaining $4,940 in funding needed for the purchase was raised through the groups Adopt-A-Bell-Drive.

  13. 2011 GRANT PROGRAM BELLEVUE RECREATION CLUB • Project Title: SPRING FLOOR & MATTING SYSTEM • Grant Amount: $ 4,798.00 • Purpose: • Provide funding towards the purchase of a 42’ x 42’ Spring Floor System which will benefit several different programs, including: • Mini cheer and competition cheer. • Biddy wrestling. • Gymnastics. • Karate. • Indoor pole vaulting instructional program. • Fundraisers have already covered $3,149 of the costs.

  14. 2011 GRANT PROGRAM BUCKEYE COUNCIL CAMPFIRE USA • Project Title: CAMP FIRE CONNECTIONS • Grant Amount: $4,596.00 • Purpose: • Provide funding towards the purchase of capital improvement items to support the restoration of previously discontinued programs, including: • Laminating equipment. • 21 Plastic folding tables and 2 table storage carts.

  15. 2011 GRANT PROGRAM FREMONT PARKS AND RECREATION • Project Title: YOUTH RUNNING PROGRAM • Grant Amount: $ 3,000.00** • Purpose: • Grant will provide funding to support the recreation departments youth running program by purchasing: • Starting blocks, numbers, shirts for events. • To supply disadvantaged youth with proper running shoes. • New hand timer and “Meet Manager” software. • Scholarship Fund – so no child gets turned away due to their economic situation. • FUNDED VIA A GRANT FROM HEINZ USA

  17. 2011 GRANT PROGRAM GOODWILL INDUSTRIES • Project Title: FREMONT PLANT PARKING EXPANSION • Amount: $ 4,800.00 • Purpose: • Provide funding for the doubling of Goodwill’s Pontiac Drive facility on Fremont’s west side. • This grant will be added to the $8,000 in funding already raised by Goodwill to cover the total cost of the project.

  18. 2011 GRANT PROGRAM HUMANE SOCIETY OF SANDUSKY COUNTY • Project Title: LIMIT THE LITTERS • Grant Amount: $ 2,500.00 • Purpose: • Provide for the spaying or neutering of cats in an effort to reduce the cat overpopulation.

  19. 2011 GRANT PROGRAM VILLAGE OF LINDSEY • Project Title: REPLACE LINDSEY LIONS COMMUNITY MESSAGE SIGN • Grant Amount: $ 10,000 • Purpose: • Provide funding for the purchase and installation of a 2-sided LED message sign. • Additional funding towards the $18,697 project is coming from: • Lindsey Lions Club. • Lindsey Tree Commission. • Lindsey PTO. • Lindsey Village Council. • Volunteer labor.

  20. 2011 GRANT PROGRAM MEMORIAL HOSPITAL FOUNDATION • Project Title: MEMORIAL PHYSICAL MEDICINE EQUIPMENT • Grant Amount: $ 8.892.96 • Purpose: • Grant will provide funding towards the purchase of equipment to support the hospital’s summer youth programs, including: • 4-in-1 climbing steps. • Triad tricycles. • Sensory stones. • Bounce path system. • iPads. • Positioning chair. • FUNDED VIA A GRANT FROM HEINZ USA

  21. 2011 GRANT PROGRAM SANDUSKY COUNTY CHILDREN SERVICES • Project Title: CHOOSE YOUR PARTNER CAREFULLY • Grant Amount: $ 2,000.00 • Purpose: • Grant proceeds will provide support for a campaign to increase awareness of the importance of partner choice and the child abuse that sometimes results from poor choices. • Funds will be used for posters, billboards, and post cards.

  22. 2011 GRANT PROGRAM VILLAGE HOUSE • Project Title: PARENTING/MENTOR CLASS PROGRAM • Grant Amount: $ 5,500.00 • Purpose: • Grant will provide funding for items to support six 6-week parenting classes will match parents with Senior Mentors. Items to be purchased include: • Workbooks. • Journals. • DVD Series. • Computer. • TV. • DVD player. • Curriculum.

  23. 2011 GRANT PROGRAM W.S.O.S. COMMUNITY ACTION COMMISSION • Project Title: EMPLOYMENT IDENTIFICATION ASSISTANCE PROGRAM • Grant Amount: $ 2,550.00 • Purpose: • Grant proceeds will be used to provide funding for birth certificates, state ID’s, and driver’s license expenses to help people obtain or retain employment.

  24. 2011 GRANT PROGRAM As the result of a gift to the Foundation from Diane (Warnke) and Hal Hawk, the Board is pleased to announce the establishment of a new scholarship. THE EARL E. WARNKE SCHOLARSHIP The scholarship will be awarded to a graduate of Port Clinton High School who was a participant in the Port Clinton Marching Band/Concert Band Program for at least three years and will be participating in a concert/pep/marching at a post secondary university of their choice.   25. 2011 GRANT PROGRAM SINCE BECOMING AN INDEPENDENT ORGANIZATION IN 2008, THE SANDUSKY COUNTY COMMUNITIES FOUNDATION, INC. HAS PROVIDED DISCRETIONARY GIFTS TO VARIOUS SANDUSKY COUNTY ENTITIES TOTALING $262,850
