15 Beacon Communities • Demonstrate a vision of the future where: • Hospitals, clinicians, & patients are meaningful users of health IT • Communities achieve measurable & sustainable improvements in health care quality, safety, efficiency, and population health Extend advanced health IT & exchange infrastructure Leverage data to inform specific delivery system & payment strategies
Beacon Grant for SE MN Vision SE MN community will embrace standards based health information exchange to improve access, quality and efficiency of health care delivery
SE MN BEACON PROGRAM A partnership with local public health and health care providers in SE Minnesota • Mayo Clinic: 1 of 17 Beacon Grant recipients nationwide through 2013 • Opportunity to enhance communication and coordination between LPH and SE MN healthcare systems • 2 demonstration projects in SE MN: childhood asthma and adult diabetes
ASTHMA AND DIABETES PROJECTS: BEACON GOALS • Laying the groundwork for better use of health information to improve health • Demonstrate secure, confidential health information exchange between parties authorized by the patient • Reduce inappropriate healthcare utilization and cost • Improve the ability of individuals to follow through on disease treatment plans
WHY IS BEACON PROPGRAM IMPORTANT TO OUR COUNTY? • $ 1.7 million investment in PH-Doc, user owned and designed local public health software operating in all 11 SE MN local public health departments • Lays the groundwork for better data about the health status of our community • Increases LPH infrastructure for more effective secure data exchange i.e. communicable diseases • Addresses two local public health responsibilities of Community Health Boards
LOCAL PUBLIC HEALTH RESPONSIBILITIES IN MINNESOTA • Promote healthy communities & healthy behaviors • Prevent the spread of infectious disease • Prepare for and respond to disasters • Protect against environmental health hazards • Assure the quality and accessibility of health services • Assure an adequate local public health infrastructure
ROLE FOR SE MN LOCAL PUBLIC HEALTH • Integrate PH-Doc IT enhancements in departments through training and practice • Coordinate with local healthcare providers in engaging school districts to participate in Beacon Program • Develop capacity for health information exchange
PARTNERSHIP WITH SE MINNESOTA HEALTHCARE PROVIDERS Beacon Program lays the groundwork for secure health information exchange between local public health departments and other stakeholders: • Health care providers • Other public health entities: state and local • School districts
ENGAGING SCHOOLS Local public health departments working with local healthcare providers to engage schools in assuring that children with asthma have best possible outcomes: “Healthy Students are Better Learners”
PARTICIPATION IN BEACON PROGRAM • January 2010 - Eleven SE Minnesota County Boards supported participation in the Beacon Communities Program • September 2010 - Next steps involve: • Incorporating IT improvements • Partnering with healthcare providers in outreach to SE Minnesota school districts
