Training Objectives • Understand the objective & the measure • Examine the use of the Patient Wellness Handout (PWH) for meeting this measure • Print the Patient Wellness Handout utilizing the EHR GUI and RPMS Roll and Scroll • Compare and Contrast the Patient Wellness Handout Tally Report (PINT) & MU Performance Measures Report • Examine policy and guidelines for Clinical Summaries Clinical Summaries and Patient Reminders
Clinical Summaries Objective and Measure Provide clinical summaries for patients for each office visit. Clinical summaries provided to patients for more than 50 percent of all office visits within 3 business days. Clinical Summaries
MU Measure Type of Measure: Rate Clinical Summaries
RPMS Reporting Logic Denominator Inclusions: COUNT: each patient face-to-face visit with a provider (Service Category of A, S, O, or M) WHERE: the visit occurred on or after the first day of the EHR reporting period AND WHERE: the visit occurred on or before the fourth business day prior to the end of the EHR reporting period Clinical Summaries
RPMS Reporting Logic Numerator Inclusions: COUNT: each patient face-to-face visit in the denominator WHERE: a Patient Wellness Handout (PWH) was generated on or after the visit date/time but within 3 business days of the visit Clinical Summaries
RPMS Reporting Logic Measure Exclusion: • EPs who have no office visits during the EHR reporting period are excluded from this measure. Clinical Summaries
Patient RemindersObjective and Measure Send reminders to patients per patient preference for preventive/follow-up care. More than 20% of all patients 65 years old or older or 5 years old or younger were sent an appropriate reminder during the EHR reporting period. Patient Reminders
MU Measure Type of Measure: Rate Patient Reminders
RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: an active health record at the beginning of the EHR reporting period AND HAVING: no date of death recorded at the beginning of the EHR reporting period HAVING: age of 5 years old and younger at the beginning of the EHR reporting period OR HAVING: age of 65 years old and older at the beginning of the EHR reporting period Patient Reminders
RPMS Reporting Logic Numerator Inclusions: COUNT: Count each patient in the denominator HAVING: one or more Patient Wellness Handouts generated during the EHR reporting period Patient Reminders
RPMS Reporting Logic Measure Exclusion: • The EP is excluded from this measure if the facility does not have any patients in the database who are: • 5 years old or younger • 65 years old or older Patient Reminders
What HIM Needs to Know • For the Clinical Summaries measure, a Patient Wellness Handout (PWH) must be generated on or after the visit date/time but within 3 business days of the visit • For the Patient Reminders measure, a PWH must be generated for preventive/follow-up care Clinical Summaries and Patient Reminders
Configuration • A Patient Wellness Handout must be created using the PWH and the VA Health Summary. • Patient Wellness Handout button must be added to the EHR GUI template. Clinical Summaries and Patient Reminders
How-to Session • Review methods for printing the PWH in RPMS and using the EHR PWH button. • Review the PWH Tally Report (PINT). • Review the MU Performance Measures Report. Clinical Summaries and Patient Reminders
