D First Step • Provision of comprehensive file records • Elapsed time since date of loss • Sufficient time must pass to allow the claimant’s health care status to have stabilized without foreseeable significant changes in status • Availability of current medical reports with opinions on probability of the development of secondary conditions • Physiatrist Neurologist • Neuro-Psychiatrist Psychologist • Orthopaedic

D Timing of The Report FCC can proceed when the following criteria is met: • 30-90 Days • Receipt of records • File review • Identification of missing information • Discussion with referral source • Meeting with claimant • Written communication with treatment team If the above criteria can not be met a File Review may be an option

D Content of report • Review Claimant’s Medical, Rehabilitation and Vocational records • Description of claimant’s status based on medical information and observation during home interview • Written recommendations from members of the treatment team • Pre-accident issues • Objective analysis of previously documented issues for accuracy, objectivity and validity
