Is transurethral resection of the prostate safe and effective in men aged over 80 years? • Rahul Bhardwaj, Luke Forster, Michael Kimuli and Sanjay Jain • St James’ University Hospital, Leeds
Introduction • Factors associated with increased TURP morbidity include resection times exceeding 90mins, gland size more than 45g, AUR and age over 80 years (Mebust et al, 1989, Gainsburg, 2008). Objective • To assess the safety and effectiveness of TURP in patients over 80 years old. Method • Evaluated men who had undergone TURP aged at least 80 years during a 36-month period. • Patient details were supplied by the coding department. • Patient’s records were reviewed electronically using Patient Pathway Manager and the result server. Demographics
Results Summary of outcomes at 4 weeks (%) Complications over 36 months
Conclusions • 85% of patients voided successfully post-operatively. • 0% 30-day mortality rate. • Morbidity rate of 29% comparable to “The National Prostatectomy Audit” rate of 35%. • High rates of patient satisfaction: 62-73%. • Age itself is not a contraindication to TURP. Age and general health don’t necessarily correlate. • Consideration of ASA is a better indicator of outcome (Kore & Blacklock, 2000).
