  8. Dental Insurance • Employee pays premium • Covered services fall into 4 categories: • Annual maximum per member - $1,000. Click here for more information.

  9. Dental Insurance • Employee pays premium • Amount covered varies depending on the service. • No In-Network deductibles • Click here for the detailed amounts. • Annual maximum per member - $2,000

  10. Vision Insurance • Employee pays Premium • Provided by Superior Vision • Coverage for annual eye exam • Coverage for frames and lenses, or contacts annually • $35 co-pay for materials and contact lens fitting

Term Life Insurance • Employee • VCSU pays premium for a $7,000 death benefit for the employee • Additional coverage from $10,000 to $400,000 • New hire – EOI required for amount above $200,000 • Dependent/Spouse • Dependent/Spouse – $2,000, $5,000, $7,000 or $10,000 option • Spouse – up to ½ of employee coverage • New hire – EOI required for amount above $50,000 Click here to view rates

Long-Term Disability • NDPERS (Bands 4000-7000) • After twelve-month eligibility period • Income of 25% of final average salary • Not reduced by other disability payments • The Standard (Bands 0000-3000) • After six-month eligibility period • Income 60-70% of monthly wage base • Reduced by other disability payments

  13. Premiums • Premiums are paid in advance of the month • 1st paycheck will have triple deductions

  16. Retirement Staff (Bands 4000 – 7000) Vesting after 3 years (36 months) Faculty, Executives, and Administrators (Bands: 0000 - 3000) *0000 job band skip the 0-2 year Supplemental Insurance Options are also available.

Flexible Spending Program & Premium Conversion • Flexible Spending • Allows employees to pay for qualifying medical expenses and/or day care expenses with tax-deferred dollars • Enrollment is on a calendar year basis • Premium Conversion • Allows employees to deduct premiums on a pre-tax basis • Yearly elections cannot be changed until the new plan year unless there is a qualifying event (birth, death, marriage, etc.)

Personal Leave • Sick Leave • Employees accrue 12 days/year at a rate of 1 day/month (prorated to FTE) • Employees may use up to 80 hours sick leave annually to assist ill immediate family • In the case of a serious health condition of an eligible family member, an employee may take up to a total of 480 hours of sick leave • Annual Leave • Eligible employees accrue annual leave from date of hire. Accrual is based on years of service: • May carry over up to 240 hours annually. Must use 40 hours/year after first year.
