Application Process for Renewal Sponsors • Simply complete a rollover process and submit annually. Most information is pre-filled from previous year’s agreement. • Application Program Year is October 1, 2005 through September 30, 2006.
Renewal Sponsors Contracting for Food Service • If you contract with a food service management company (FSMC), vendor or another school district, please submit a copy of the contract or agreement to our office as soon as possible. • Your sponsor application will be in Pending status until the appropriate paperwork is reviewed by our office.
Sponsor & Site ApplicationsRequiring Changes • You are responsible to keep information up to date in Child Nutrition ACES! • Sponsor or Site Name and Address • Authorized Representative and Contact Person Information • Months of Operation • Program Participation
NSLP Addendum • Information is specific to the last operating day in October • Required fields for each NSL Site • The total number of students enrolled who have access to the school lunch program • Total number of students eligible for free lunches • Total number of students eligible for reduced-price lunches • Grade levels where students have access to the NSLP
Verification Summary Report • Verification process must be completed no later than November 15th annually. • The verification summary report must be submitted electronically through Child Nutrition ACES to ISBE no later than January 18th.

Reporting Categories • Each category asks for number of students and number of approved applications • Total Free Eligible • # approved who are not subject to verification (This item includes the direct certification eligibility certificates) • # approved based on FS/TANF number submitted on an application (Categorically) • # approved based on income/household size submitted on an application (Income) • Total Reduced-Price Eligible
Child Nutrition Annual Financial Report (AFR) • Completed annually to document nonprofit food service status • Deadline for submission of the FY2005 Child Nutrition AFR was October 17, 2005 • Reporting period is July 1, 2004 through June 30, 2005 • Completed electronically through Child Nutrition ACES
