  3. General Instructions • Grant application (and six copies) must be received at Council office by 5:00 p.m., Friday, March 2, 2012. This includes the electronic submission via the DD Suite AND the hard copy with original signatures. • Both submissions must meet deadline. Late applications will be unconditionally refused. • Applicant must submit original hard copy application and six copies. • Hard copy applications must be double spaced, size 12 font, and stapled in upper left corner. Do not submit applications in binder clip or cover. • All applications must follow Council format, as outlined in application instructions. • Applicants must complete a separate application for each Request for Application.

  7. Project Profile • Specify the RFA that is being applied for • Provide a Title of the Project • Applicant Agency • Organization that will receive grant award. Must be a legal entity (public agency, private not-for-profit, private-for-profit). • Project Director • The person responsible for carrying out the terms of the grant. • Organization Director/Authorized Official • The person having legal authority to enter into binding contracts determined by applicant agency.

  8. ProjectProfile(cont.) • Implementing Agency • Organization responsible for carrying out the terms of the grant application. (Must disclose any sub-contractor). • Proposed Financial Information • Proposed costs of first year of project (consistent with Budget Summary).

  9. Project Outline Form • Preferred: A brief concise paragraph for each “topic”. • 2500 Character Limit -Continue narrative on attachment.

Project Outline Form (cont.) • Plan for dissemination of materials • Council approval is required prior to dissemination. • Acknowledgement of Council support on all publications (See Assurances). • Post Project Activities • As determined by the design of the project, describe plans for continuation activities – including, identification of resources for continuation.

Proposed Work Plan • Objective • List each objective of the project separately. • Activity • Define the activities necessary to achieving the objective. • Timeline • Projected start and completion date for each activity. • Responsible Person • Identify each person responsible for completing an activity.

Evaluation Strategy(There is no form) • All applications must include internal evaluations consisting of: • Process for gathering information • Process for analyzing information • Identification of individuals conducting and participating in evaluation • Plan for utilizing and communicating results

  27. Endorsements(There is no form) • Letters of endorsements and support indicating coordination/collaboration from agencies and organizations at the state and community level in this project and other activities

  28. Grant Awards Both of these grant awards will be for up to $75,000 for up to 24 consecutive months with a $25,000 match and intended to begin, July 1, 2012.   29. Award Process • Review team will make recommendation to the NCCDD • NCCDD will determine grant award at May Council Meeting • Implementation of grant will be July 1, 2012
