Purpose of Documentation Communication / Evidence /Research /Education /Quality Assurance/ Statistics / Reimbursement • Proof of facts and records • Regulatory requirements • Standardization • Improve performance, efficiency and quality • Understand end user requirements • Use as a reference • Creates resource for learning and knowledge sharing Do we document everything? All organizations have a purpose to meet the needs & expectations of their clients/customers and have phases in their life cycle with the focuson: • Maintenance of the Operations • Improvement of Operations • Organizational Development MAINTENANCE • Budgeting • Review of Policies, Rules & Regulations • Review of Operating Procedures • Program Review & Planning IMPROVEMENT • Problem Solving & Systems Analysis • Continuous Quality Improvement • Best Practices DEVELOPMENT • Strategic Planning  • Restructuring & Re-Engineering  • Innovation

Best Practices • A method or technique that has consistently shown results superior to those achieved with other means, and that is used as a benchmark. • Through experience and research, has proven reliably to lead to a desired result Best Practices are used for: • Improving public policy based on what works; • Raising awareness of decision-makers at all levels and of the general public on potential solutions to common social, economic and environmental problems; • Assessing emerging issues, trends and policy responses; • Sharing and transferring expertise and experience through networking and peer-to-peer learning.

  7. Criteria - Best Practice Effectiveness: Achieve measurable results. Efficiency: Produce results with a reasonable level of resources and time. Relevance: Address the priority problems in the region. Ethical soundness: Respect the current rules of ethics for dealing with human populations. Sustainability: Implementable over a long period of time without any massive need for additional resources. Possibilityof Duplication: Must be replicable : elsewhere in the region. Involvement of Partnership: involves collaborative partnerships: between several stakeholders. Community Involvement: Involves participation of the affected communities. PoliticalCommitment: Have support from relevant committed national or local authorities

Template – Best Practice Introduction This should provide the context and justification for the practice and address the following issues: • what is the problem being addressed? • which population is being affected? • how is the problem impacting the population? • what were the objectives being achieved? Implementation of the Practice • what are the main activities carried out? • when and where were the activities carried out? • who were the key implementers and collaborators? • what were the resource implications?

  10. Results of the Practice – Outputs and Outcomes • what were the concrete results achieved in terms of outputs and outcomes? • was an assessment of the practice carried out? If yes, what were the results? Lessons Learnt • what worked really well – what facilitated this? • what did not work – why did it not work? Conclusion • How have the results benefited the population? • Why is the intervention considered a “Best Practice”? • Recommendations for those intending to adopt the documented “Best Practice” or how it can help people working on the same issue(s). Further Reading • Provide a list of references that give additional information on the “Best Practice” for those who may be interested in how the results have benefited the population
