MAPP Assessments Overview • Four MAPP Assessments • Community Themes and Strengths • “What is important to our community?” • Local Public Health System • “How are the Essential Services being provided?” • Community Health Status • “How healthy is are our residents?” • Forces of Change • What is occurring or might occur that affects the health of our community or LPHS?”
Community Health Status Assessment • METHOD: • Collect, analyze and review public health data • Describe population health status • Identify health issues by assessing • Changes over time • Differences among groups • USES: • Develop priorities • Plan actions to improve health outcome • Assists public health and emergency mgmt in readiness/preparedness plans
Step 1 – Prepare for the Assessment • Establish subcommittee • Planning considerations • Include individuals with some experience or expertise in data analysis and interpretation • Not ad hoc committee, work in monitoring will continue
Step 2 – Review of Data • Identify locally relevant indicators • Collect and analyze data • Use data from previous assessments • Access data from partner agencies • Review trend data • Examine data over time • Note direction (increase/decrease) • Comparison data • Goals/Benchmarks – HP2020 • Place – State, County, Region • Person – Age, Gender, Race,

Step 3 – Organize and Present Findings in Community Health Profile Report • Elements: • Narrative, data tables, comparison graphs, trend lines, maps, etc. • Priority issues identified by community • Uses • Discussion • Strategizing • Action planning • Advocacy
